Provider End User License Agreement

This agreement is between Menehune Health and you as a healthcare provider using the platform. It exists to make one thing unmistakable: the platform is a tool that organizes and transmits YOUR professional records — you remain the author of, and the party responsible for, every fact in them.

1.What you are agreeing to

By creating a provider account or continuing to use one, you accept this agreement, the Terms of Service, and the Privacy Policy. This document adds the provider-specific rules; where it conflicts with the Terms of Service, this document controls for provider accounts.

2.Your license to use the platform

We grant you a personal, limited, revocable, non-transferable license to use Menehune Health for managing your own credentials, applying to positions, and working placements arranged through the platform. You may not share your account, scrape or bulk-export data that is not yours, probe or bypass access controls, or use the platform on behalf of any other person or business.

3.Your credential file is your attestation

Every document you upload, every field you confirm, and every form you sign through the platform is your own professional representation. You are responsible for its accuracy and completeness. Uploading someone else’s records, altering documents, or letting inaccurate information stand after you have seen it may constitute fraud against the facilities that rely on it — and is grounds for immediate termination of your account.

4.Automated reading is an assistant, not an authority

The platform uses automated tools — including AI models — to read your documents, sort vaccines into records, extract dates and license numbers, and pre-fill forms. These tools make mistakes. The product shows you what it read and asks you to review before anything is submitted or signed; what you confirm becomes your statement, not the software’s. You agree to review extracted values before relying on them, and you agree that Menehune Health is not liable for consequences of extraction errors you had the opportunity to review.

5.Verification data is reported, not guaranteed

License checks (including NURSYS and state-board data), federal-registry sweeps (NPPES, OIG, SAM.gov), and similar features relay what primary sources report at the time of the check. We do not certify licensure, employability, or compliance; facilities make their own credentialing decisions. A “verified” indicator means a source responded — it is not a promise about your standing.

6.No medical, legal, or employment advice — and no guarantee of work

Nothing on the platform is medical, legal, tax, or career advice. We do not guarantee interviews, offers, placements, hours, or income. Facilities are independent businesses; your engagement terms are between you and them.

7.Electronic signatures

You consent to transact electronically. Signatures you apply through the platform are intended to be binding under applicable e-signature law (including ESIGN and UETA), and the platform records document integrity (including cryptographic hashes) at signing.

8.Indemnification

You agree to defend and hold harmless Menehune Health from claims arising out of inaccurate or falsified information you provide, your professional conduct at any facility, or your breach of this agreement.

9.Disclaimer and limitation of liability

The platform is provided “as is” without warranties of any kind, express or implied. To the maximum extent permitted by law, Menehune Health’s total liability to you for all claims arising out of the platform is limited to the greater of the amounts you paid us in the twelve months before the claim or one hundred U.S. dollars ($100); and neither party is liable for indirect, incidental, or consequential damages.

10.Termination, governing law, changes

You may stop using the platform at any time; we may suspend or terminate accounts that breach this agreement. This agreement is governed by the laws of the State of Hawaiʻi, without regard to conflict-of-law rules. When we change this document substantively we bump its version and ask you to accept again before continuing; your acceptance history is kept with the version you accepted.
